Useful Tools for Aging with Dignity
In three workshop sessions, participants will learn tools that aid in life planning and encourage in-depth conversations about health with the people in their lives.
Full of tools, strategies, and checklists on how to stay connected to community, these workshops are appropriate for adults of all ages.
Session One — Calendar of Seasonal Living. Participants learn how to create a calendar that presents ideas and questions relevant to each calendar month, encouraging participants to proactively schedule seasonal activities to stay connected with their community.
Session Two — A Checklist of Resources to Aid Life Care Planning. Participants learn to create a comprehensive list of essential information that can be used by the person(s) who will be responsible for managing or helping to manage their affairs.
Session Three — Honoring Care Decisions. Participants receive fact sheets and guidance based on the Honoring Care Decisions approach developed by Dartmouth Hitchcock Medical Center. Participants also receive both the NH and VT Advance Care Plan (ADC) forms, and learn how to document their wishes with their Health Care Agent.
